By typing my name below, I give permission for my son/daughter, listed below, to participate in the St. Mary VBS Service projects that take place in the Solon Community.

​I understand that my child will walk or be transported by an adult, from my parish.  I release St. Mary Parish- Solon, IA and all adult chaperones from any and all claims arising out of or from any accident or other occurrence, causing injury to any person or property during this event. ​
