FOR FIELD TRIP PARTICIPATION
Dear Parent or Legal Guardian:
Your son/daughter, legal dependent, is eligible to participate in a school-sponsored or Religious Education Program activity at a location away from the school/parish site. This activity will take place under the guidance and supervision of St. Mary’s VBS Program. A brief description of the activity follows:
Activity: VBS 6th Grade Service Trips
Destination:
Solon Food Pantry & Senior Dining - learn about these services (July 16)
Solon Nursing Care Center – visit with residents & to pray the rosary at mass (July 17 & 18)
Designated Supervisor: Tracy Rusch
Transportation provided by: 6th Grade Parents to Solon Care Center on July 17 & 18; students will walk to other events.
Place of Departure & Return: St. Mary Church
Please drop off / pick up at regular times on those days 8:30-11:30
If you would like your child to participate in this event, please complete and submit the online statement of consent below. As parent, or legal guardian, you remain fully responsible for any legal responsibility that may result from any personal actions taken by the named student.
