By typing my name below, I give permission for my daughter, listed below, to participate in the St. Mary Parish Girl's Lock in to be held on Tuesday, July 12 from 11:30am to 9:00pm on parish grounds and locations around Solon. In the event of sickness or accident, the adults supervising the Lock-In have my permission to secure medical care for my child. I understand that my child will be transported by an adult from my parish. I release St. Mary Parish- Solon, IA and all adult chaperones from any and all claims arising out of or from any accident or other occurrence, causing injury to any person or property during this event.
